CHN Program Theory
An evidence-based program theory was developed based on literature and is being revised based on data from the ENCOMPASS studies. ENCOMPASS (ENhancing COMmunity health through Patient navigation, Advocacy, and Social Support) was a research partnership between the University of Calgary, University of Alberta, and multiple Primary Care Networks (PCNs) to study the impact of a patient navigation intervention for patients with chronic disease. The program theory, in its present form, is depicted below and describes CHN activities, anticipated outcomes, and mechanisms of action.
